Job Title: Medical Social WorkerJob Type: Part-timeLocation: Tucson, AZSalary Range: $60 to $65 per hourJob Overview:As a Medical Social Worker at Essence Hospice Care of Arizona, you will play a vital role in providing compassionate care and support to patients facing end-of-life challenges. Working closely with our medical teams, you will coordinate treatments, services, and resources to enhance the quality of life for our patients. Your expertise in counseling and referral services will be essential in guiding patients and their families through this difficult time.Responsibilities:- Collaborate with medical teams to develop and implement care plans for patients- Assess patients' psychosocial needs and provide counseling and support- Coordinate with community resources to assist patients with financial, emotional, and practical challenges- Advocate for patients' rights and ensure their wishes are respected- Maintain accurate and timely documentation of patient careRequirements:- Master's degree in Social Work from an accredited program- Current state licensure as a Social Worker- Minimum of 2 years of experience in a healthcare setting, preferably in hospice or palliative care- Strong communication and interpersonal skills- Ability to work effectively in a multidisciplinary team- Compassionate and empathetic approach to patient carePlease note: Essence Hospice Care of Arizona is an equal opportunity employer and complies with all applicable laws regarding hiring practices.
