Physical Therapist Qualifications
  • Active Physical Therapist license in North Carolina
  • Minimum 1 year of experience as a Physical Therapist
  • At least 6 months of recent experience in a Home Health setting
  • Proficiency with OASIS and HCHB documentation systems
  • Current CPR certification
  • Valid driver's license, vehicle insurance, and reliable transportation
Home Health Physical Therapist Responsibilities
  • Provide skilled physical therapy services to patients in their homes throughout Waynesville and surrounding areas
  • Conduct comprehensive patient assessments and create individualized care plans
  • Accurately document patient progress using OASIS and HCHB systems
  • Collaborate with an interdisciplinary healthcare team to improve patient outcomes
  • Educate patients and families on therapy techniques and home exercise programs
  • Maintain compliance with all state and federal healthcare regulations
