The Physical Therapist (PT) is responsible for providing exceptional, patient-centered care by managing therapeutic processes for an assigned group of patients, collaborating closely with physicians and care teams. This role involves conducting patient assessments and developing tailored care plans that prioritize patient engagement and positive outcomes. As a Physical Therapist, you will deliver most of the therapeutic care, guiding patients throughout their recovery journey.

Key Responsibilities:

• Evaluate and implement treatment interventions within established professional guidelines.
• Assess and document therapeutic and functional status while contributing to the holistic care plan.
• Deliver hands-on therapeutic care in patients' homes, using evidence-based techniques that align with medical guidance.
• Educate patients, families, and caregivers on relevant therapies, exercises, and assistive devices.
• Manage your caseload efficiently, coordinating daily staffing needs and patient transitions.

Qualifications:

• Must obtain a licensed Physical Therapist designation within 30 days of employment start date.
• Bachelor's degree in relevant field.
