The Opportunity
- Full control over your calendar with a customizable schedule.
- Support from a dedicated personal scheduler.
- Flexible work schedule that suits your lifestyle, Monday through Friday (predictable weekends).
- No on-call responsibilities.
- Serve the needs of communities in southwestern WI and northern IL.
- Utilize EPIC EMR for efficient documentation.
- Manage an established patient panel—no need to build from scratch.
Your Responsibilities
- Provide exceptional care in nursing homes across the Monroe and Freeport areas.
- Lead a passionate team of 3 specialized Geriatric Nurse Practitioners.
- Be part of a close-knit, mission-driven healthcare community.
- Enjoy high levels of autonomy and independence in your practice.
Your Qualifications
- Board certification in Family Medicine or Internal Medicine.
- Ability to obtain dual licensing in WI and IL.
- Preferred experience in post-acute and long-term care.
- A genuine passion for patient-centered, compassionate elder care.

Salary Range: $135,000.00 to $857,000.00 / Year
