The Opportunity Full control over your calendar with a customizable schedule. Support from a dedicated personal scheduler. Flexible work schedule that suits your lifestyle, Monday through Friday (predictable weekends). No on-call responsibilities. Serve the needs of communities in southwestern WI and northern IL. Utilize EPIC EMR for efficient documentation. Manage an established patient panel—no need to build from scratch. Your Responsibilities Provide exceptional care in nursing homes across the Monroe and Freeport areas. Lead a passionate team of 3 specialized Geriatric Nurse Practitioners. Be part of a close-knit, mission-driven healthcare community. Enjoy high levels of autonomy and independence in your practice. Your Qualifications Board certification in Family Medicine or Internal Medicine. Ability to obtain dual licensing in WI and IL. Preferred experience in post-acute and long-term care.
