POSITION SUMMARY:

The Population Health Nursing Specialist plays a pivotal role in overseeing and improving clinical workflows within healthcare practices to achieve PCMH level 3 recognition. This position involves identifying and managing a population of individuals with specific healthcare needs through a tailored stratification process that leverages data from various sources, including patient visits, claims, lab results, and hospital admissions/discharges.

You will collaborate with clinical staff, physicians, and quality committees to enhance patient outcomes, particularly for those with chronic diseases, by ensuring timely and appropriate healthcare coordination.

EDUCATION/CERTIFICATION:

  • Graduate of an accredited nursing program.
  • Associate degree required; bachelor’s degree preferred.
  • Current Licensed Practical Nurse license in Connecticut is mandatory.

EXPERIENCE:

A minimum of five (5) years of nursing experience in a community health setting, particularly working with vulnerable populations, is essential.

COMPETENCIES:

  • Strong analytical and organizational skills
  • Interpersonal skills with effective communication abilities in English, both verbal and written
  • Proficient in Microsoft Office applications
  • Ability to multitask, work independently, and demonstrate initiative

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Collaborate with physicians and staff to identify vulnerable patients, in alignment with PCMH standards.
  • Act as a resource for referral departments on clinically-required authorization calls.
  • Support practices lacking LPN staff, including assisting with treatment care plan development.
  • Review and implement essential PCMH processes within physician offices.
  • Engage in advanced research for future PCMH standards.
  • Provide ongoing practice support to ensure adherence to clinical standards.
  • Identify clinical quality initiatives and facilitate their implementation across practices.
  • Participate in regular team and peer review meetings, and join departmental committees as necessary.
  • Foster a collaborative team environment and maintain patient confidentiality adhering to HIPAA guidelines.
