General Summary of Position
As a Clinical Pharmacist, you will play a vital role in providing pharmaceutical care to patients, closely collaborating with a senior clinical pharmacist or manager. You will evaluate physician orders and dispense medications while ensuring compliance with operational procedures, professional standards, and legal requirements.
Primary Duties and Responsibilities
- Assess medication orders for accuracy and resolve discrepancies through effective communication with physicians and nursing staff.
- Evaluate and compound sterile products and prepare IV admixtures, including TPN solutions and cytotoxic agents.
- Respond to non-evaluative drug information inquiries independently.
- Monitor patient drug therapy and ensure optimal therapeutic outcomes under supervision.
- Review patient profiles for contraindications and ensure appropriate drug selection considering the patient's physiological status.
- Dispense controlled substances while adhering to legal and departmental policies.
- Supervise support personnel, oversee workflow, and ensure preparation accuracy.
- Maintain accurate computer entries for drug orders and counsel patients on medication safety and potential side effects.
- Stay updated on current pharmacy trends through literature, seminars, and continuing education.
- Participate in departmental education programs and contribute to interdisciplinary quality improvement teams.
- Engage in community outreach and represent the pharmacy department in meetings and committees.
Minimal Qualifications
Education
- Doctoral degree in Pharmacy or equivalent from an accredited institution is required.
Experience
- 1-2 years of direct patient care experience in a hospital setting is required; PGY-1 residency is preferred.
Licenses and Certifications
- Registered Pharmacist (RPH) licensure in the state of practice is required upon hire.
- Board certification is preferred upon hire.
Knowledge, Skills, and Abilities
- Strong problem-solving skills to address complex clinical, technical, and administrative challenges.
- Effective communication skills to advise, consult, and gain cooperation from others.
- The ability to exercise independent judgment in analyzing problems and forming solutions.
- Basic computer skills are preferred.
The compensation for this position ranges from USD $102,606.00 to USD $196,996.00 per year.
