As a Revisit LPN, you will leverage your education and skills to provide personalized and compassionate care to our patients in an environment that fosters independence.

Key Responsibilities:

  • Facilitate communication between patients and the interdisciplinary team.
  • Build and sustain therapeutic relationships by educating patients and encouraging self-advocacy.
  • Utilize advanced technology to document the skilled services provided effectively.

Qualifications:

  • Licensed Practical Nurse with a valid CT LPN license.
  • At least one year of LPN experience is required; homecare experience is a plus.
  • Must have a valid driver’s license and reliable transportation.
  • Computer proficiency is essential.
