Key Responsibilities:

  • Facilitate effective communication between patients and the interdisciplinary care team.
  • Develop and maintain therapeutic relationships to empower patients through education, promoting self-advocacy and independence.
  • Utilize advanced technology to accurately document the skilled services you provide.

Qualifications:

  • Licensed Practical Nurse with a valid CT LPN license.
  • A minimum of one year of LPN experience; home care experience is a plus.
  • A valid driver's license and reliable transportation are required.
  • Proficiency in computer skills is essential.
