POSITION DUTIES

The purpose of the Infection Control and Linkage to Care Coordinator position is to assist with monitoring the contractors' Infection Control Program. This position will assist with monitoring the quality and standard of healthcare provided by contractors for compliance with requirements enumerated in DPSCS contracts and policy as it pertains to screening, testing, treatment and reporting of all infectious diseases; review the quality of care at facilities via the quality improvement audit process; and work with the contractors to ensure accurate monthly submission of infectious disease data.

This position requires regular use of independent judgment and analysis in applying and interpreting complex administrative plans and/or policies. This position will act as the department liaison with MDH, Baltimore City Health Department and other Local Health Departments on issues surrounding HIV and HCV, training, policy and procedure, data development and management and other reportable infectious diseases.

This position is also responsible for supporting the development and implementation of EPHR data collection systems to assist with reporting and quality improvement recommendations related to infectious diseases. This position will work closely with ITCD to ensure appropriate documentation is available to assist with this effort.

This position will work collaboratively with the department's Agency Contract Operations Managers (ACOMs), Continuous Quality Improvement (CQI) Nurses, and the Death Review Nurse in the oversight of the Infection Control Programs and shall follow through with request for corrective actions and liquidated damages assessment for contract non-compliance.

  • Infection Control Program Monitoring and Reporting - Develop data collection and audit tools for tracking infectious disease data and for continuous quality improvement and contract compliance monitoring related to the Infectious Disease Program. Reviews sharepoint monthly to confirm contractors' submission of all contractually required infectious disease reports and follows through with contractors for corrective actions and liquidated damages assessment when indicated. Work with the medical contractor's discharge planners to ensure linkage to care for all individuals with infectious diseases released to the community. Participates in the development of infectious disease documentation and data collection tools in the EPHR.
  • Agency Representation and Data Management - Serve as the department's liaison with MDH; County and City Health Departments on all activities and committees for viral Hepatitis, HIV and all other reportable diseases. Participates in DPSCS committees and at various vendor meetings as requested by the CQI/IC Nurse Manager or other Clinical Services leadership. Collaborates with the contractor to collect and submit monthly infectious disease data and provides a monthly summary report of infectious diseases to the CQI/IC Nurse Manager. Presents data at the bi-monthly statewide infection control meetings as needed to support the Infection Control Program.
  • Grant Oversight and Research Oversight - Oversee all current and future research projects and grants on viral hepatitis, HIV and other infectious diseases and provide monthly reports to the Chief Medical Officer and Deputy Director of Clinical Services.


MINIMUM QUALIFICATIONS

Education: Master's degree in any health related field.

Experience: At least two years of experience monitoring an infectious disease program- including data collection, analysis, and reporting via spreadsheets.

DESIRED OR PREFERRED QUALIFICATIONS
• Preference will be given to applicants with a Master's Degree in Public Health.
