Minimum Qualifications:
- Minimum of 3 years of clinical experience in nursing or a related healthcare field, preferably in community health or social assistance settings.
- At least 2 years of supervisory or leadership experience in a clinical environment.
- Strong knowledge of medical respite care principles, patient care standards, and healthcare regulations.
- Excellent communication, organizational, and interpersonal skills.
- Experience working with vulnerable populations, including individuals experiencing homelessness.
- Familiarity with case management, care coordination, and trauma-informed care approaches.
- Proficiency with electronic health records (EHR) systems and data reporting tools.
- Supervise and provide clinical guidance to healthcare professionals within the Medical Respite Program.
- Coordinate interdisciplinary team meetings to review patient progress, address challenges, and plan individualized care strategies.
- Ensure compliance with healthcare regulations, accreditation standards, and organizational policies.
- Provide direct clinical support and consultation for complex patient cases as needed.
- Facilitate staff training, professional development, and performance evaluations to maintain a skilled and motivated clinical team.
- Collaborate with community partners, healthcare providers, and social service agencies to support patient transitions and resource linkage.
Skills: The Respite Clinical Supervisor utilizes clinical expertise and leadership skills daily to guide and support the team in delivering patient-centered care. Strong communication skills are essential for coordinating interdisciplinary collaboration and effectively engaging with patients and community partners. Organizational and problem-solving abilities enable the supervisor to manage complex cases, oversee program operations, and ensure compliance with healthcare standards.
