Lead Case Manager- Assertive Community Treatment

Location: St. Paul, MN
Purpose:
The Lead Case Manager is responsible for providing comprehensive, community-based services to individuals with severe and persistent mental illness (SPMI) and chemical dependency on Guild's Assertive Community Treatment (ACT) team. In addition to delivering direct client support, managing medications, assisting with benefits, and coordinating care to improve client functioning and quality of life, the Lead Case Manager plays a key leadership role. This includes supporting the Program Manager and Clinical Supervisor with administrative duties such as organizing and managing the team's calendar, facilitating team meetings, providing guidance to case managers, and assisting with training and development. The Lead Case Manager ensures efficient team operations while maintaining high standards of care and client service.

- Build trusting relationships with clients to encourage their active participation in treatment and recovery.
- Assist clients in managing their mental health symptoms, daily living skills, and overall well-being.
- Collaborate with other service providers and natural supports, ensuring clear communication and coordination of services.
- Actively participate in team meetings and care conferences to discuss client progress and share insights with team members.
- Complete required documentation in compliance with organizational and regulatory standards.
- Lead complex client cases (e.g., hospitalizations, inactive insurance, loss of benefits, criminal records, transitions to assisted living/residential placement) in situations where primary staff are unavailable or require additional support.
- Oversee the completion of civil recommitments and revocations, ensuring all necessary documentation and procedures are followed in a timely manner.
- Perform other duties as assigned to support client care and department operations.
- Promote an inclusive, diverse, equitable, and respectful work environment.
- Understanding of mental health diagnoses, treatment modalities, and the specific needs of individuals with SPMI.
- Understanding of community resources, county benefits, and mental health services.
- Ability to manage a caseload, prioritize tasks, and meet deadlines in a dynamic, community-based environment.
- Ability to assess and intervene in crisis situations, utilizing de-escalation techniques and connecting clients to higher levels of care when needed.
- Ability to develop and maintain receptive, open, and approachable lines of communication with clients and staff, and provide a friendly environment for visitors.
- Strong written and verbal communication skills, including composing, proofreading, and editing.
- Presentation and meeting facilitation skills.
- Ability to work as a proactive and positive member of a team to deliver quality services.
- Ability to maintain confidentiality of organization, personnel information, and client data.
- Ability to use sound judgment and maintain healthy professional boundaries.
- Analytical and problem-solving skills.
- Proficiency with a variety of web-based software and databases, including Microsoft Office Suite.
- This position is primarily community-based, requiring frequent travel to clients' homes and various community locations.
- Prolonged periods at a desk, working on a computer.
- Walking and standing continuously for hours at a time.
- Manual dexterity for tasks such as taking notes, using technological devices for documentation, or assisting clients with certain activities if needed.
- Must be ambulatory and able to negotiate stairways.
- The ability to lift and carry heavy weights that could be up to 50 pounds.
Required
- Bachelor's degree in one of the behavioral sciences (social work, psychology, or human services) or a related field from an accredited college or university AND have 2,000 hours (1 year) of experience in the delivery of service to persons with mental illness OR
- Qualified as a mental health practitioner within the State of Minnesota by one of the following:
- At least 30 semester/45 quarter credits AND 2,000 hours of supervised experience in the provision of mental health services. OR
- 4,000 hours of supervised experienced in the provision of mental health services.
- Master's degree in one of the behavioral sciences (social work, psychology, or human services) or a related field from an accredited college or university.
- Demonstrated experience providing Assertive Community Treatment (ACT) services.
- Professional experience working in a position with leadership responsibilities.
- Must not be actively receiving services from Guild and must not have received services within the last two years.
- A valid Class 'D' Driver's License, reliable vehicle, satisfactory driving record and necessary insurance.
- Available to provide on-call crisis support 24/7 in weekly intervals on rotation.
- Occasional evening or weekend work.

The rate range for this position is between $62,400-$66,000 annually. The range reflects the minimum and maximum salary for the position. Compensation will depend on the candidate's professional experience, skills, and relevant education, training, and certifications.
