HCBS Intake Coordinator

Doral Health & Wellness is seeking a dedicated and detail-oriented Intake Coordinator to join our growing team. This full-time, in-person role is ideal for someone who is organized, compassionate, and experienced in supporting families navigating Home and Community-Based Services (HCBS).
As an Intake Coordinator, you will be the first point of contact for clients and families. You will guide them through eligibility, documentation, and the intake process while ensuring a smooth and supportive experience.
Compensation
- Pay Rate/Salary: $25 per hour

Key Responsibilities
- Serve as the primary Intake Coordinator for clients applying for HCBS programs.
- Conduct intake screenings, collect required documentation, and verify eligibility for HCBS services.
- Communicate clearly and professionally with clients, families, and providers.
- Process referrals and maintain accurate records in the EHR/EMR system.
- Coordinate with internal teams to ensure timely scheduling and service initiation.
- Provide ongoing support and follow-up to ensure successful onboarding.
- Maintain confidentiality, compliance standards, and program guidelines.
Qualifications
- High School Diploma or GED required.
- English required; Spanish preferred.
- Prior intake coordinator or similar intake experience strongly preferred.
- HCBS experience highly preferred (Children’s HCBS or similar programs).
- Strong communication, customer service, and organizational skills.
- Ability to work independently in a fast-paced, office-based environment.
- Proficiency with computers and electronic health records.
