Responsibilities

The Nurse Practitioner will conduct annual health risk assessments in residential settings or via telehealth, adhering to the scope of practice as designated by the Collaborative Physician.

Essential Duties and Responsibilities

  • Conduct thorough in-home health risk assessments to identify active and chronic diseases and assess physical, mental, and social needs during patient visits.
  • Gather patient history, perform examinations, and establish diagnoses.
  • Document visit outcomes in writing, in line with NCQA standards.
  • Measure patient vital signs as needed.
  • Facilitate case management referrals and coordinate with PCPs as required.
  • Communicate effectively with patients, caregivers, agency nurses, and various healthcare providers.
  • Follow OSHA Universal Precautions during all clinical duties.
  • Maintain patient confidentiality at all times.
  • Attend required meetings, in-services, and participate in committees as requested.
  • Engage in professional development to sustain licenses and affiliations.

Qualifications

Required Knowledge, Skills, and Experience

  • Valid, unrestricted nurse practitioner license to practice.
  • Board certification through ANCC, AANP, or NCCPA.
  • Current Basic Life Support (BLS) certification.
  • Enrollment in Medicare/Medicaid systems.
  • Valid driver's license and a clean driving record.
  • Strong electronic health record (EHR) skills.

Preferred Knowledge, Skills, and Experience

  • Experience or training in geriatrics.
  • Strong teamwork skills and the ability to foster effective working relationships.

Physical and Environmental Conditions

  • Ability to adapt to varying weather conditions and residential environments.
  • Full range of motion and dexterity, including lifting patients.
  • Regularly lift and carry items weighing up to 50 pounds.
  • Ability to travel up to 150 miles daily in various settings.
