Job Title: Registered Nurse Profession: Nursing Specialty: Home Health Duration: 13 weeks Shift: Day Hours per Shift: 8 Experience: Minimum 1 full year Adult Home Health Experience or 1 full year Hospice experience. License: Must have license in hand. Certifications: CPR, BLS Must-Have: Must have recent specialty experience. Must be proficient in HomeCare HomeBase (HCHB) and have used it within the last 6 months. Must be proficient in OASIS. Must have reliable transportation, a current driver's license, and agency-required liability insurance. Description: Provides, coordinates, and directs the provision of home nursing care according to physician orders based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses/Licensed Vocational Nurses. Home visits are required, along with periodic office meetings. Must be willing to travel up to 75 miles from the home clinic. Mileage is reimbursed at the IRS standard rate. Contractor will be assigned between 5-10 patients per day. Patients are expected to equate to roughly 6-8 points daily. Types of cases include Start of Care, Wound Care, and follow-up visits. All documentation must be completed within 24 hours of the appointment with the patient. Delayed and incomplete documentation that impacts billing and follow-up patient care will result in cancellation of assignments. Weekly productivity points expectation is 30 points, which equates to approximately 40 hours per week. All expected patient visits are to be accepted regardless of distance from previous appointments. There are no guaranteed hours with this contract. All overtime must be pre-approved. Local candidates are accepted. Must be open to on-call rotations.
