Shift Detail: Mon-Friday 8am-4:30pm with covering one weekend per month and one major and two minor holidays per year, covering Naugatuck, Beacon Falls, Bethany, Southbury, Woodbury, Middlebury, Watertown.

As a Revisit LPN, you will leverage your education and training to deliver personalized care in a fulfilling and autonomous environment.

Key Responsibilities:

  • Facilitate communication between patients and the interdisciplinary team.
  • Develop and maintain therapeutic relationships, providing education and promoting self-advocacy and independence.
  • Utilize advanced technology to document skilled services effectively.

Qualifications:

  • Licensed Practical Nurse with a valid CT LPN license.
  • Minimum of one year LPN experience; homecare experience preferred.
  • Valid driver's license and reliable transportation required.
  • Proficient in computer use.
