The Gardens at West Shore is seeking a compassionate and dedicated Social Services Assistant to join our team. In this important role, you will support residents and their families by advocating for their social and emotional well-being while assisting in the coordination of the Social Services Department.
Responsibilities
- Assist in developing individualized care plans addressing residents' psychosocial and emotional needs.
- Support the admission and discharge process, ensuring smooth and supportive transitions.
- Provide emotional support through individual and group interactions during periods of adjustment or crisis.
- Maintain accurate, detailed documentation of residents' behavioral and psychosocial status.
- Assist in coordinating volunteer programs and community engagement initiatives to enhance the quality of life.
- Educate and orient residents and families on facility services, policies, and available resources.
- Bachelor's degree in Social Work or a related field.
- Experience in long-term care or a healthcare setting preferred.
- Strong knowledge of federal and state regulations governing social services in skilled nursing facilities.
- Excellent communication, organizational, and interpersonal skills.
