The Peer Recovery Support Specialist is an individual who is in recovery from substance use disorder. Their life experiences and recovery allow them to provide recovery support in such way that others can benefit from their experiences. Functions as a role model to peers, exhibiting competency in personal recovery and use of coping skills; serves as a consumer advocate, providing consumer information and peer support for patients.
Major Responsibilities:
- Serves as Peer Recovery Support Specialist (PRSS). Provides individual screening, patient engagement, warm hand offs, and brief therapeutic services utilizing a solution focused, therapy framework.
- Establish relationships with patients while screening for Opioid Use Disorder (OUD)/Substance Use Disorder (SUD). While doing this, PRSS can share his/her lived experience of OUD/SUD and recovery
- Work closely with the OUD team with patients to assess the desired level of care after discharge
- Collaborate with the team to coordinate referrals to inpatient or outpatient treatment facilities
- Visit daily with inpatients to conduct motivational interviews and provide psychosocial support.
- Collaborate with the OUD/SUD team to ensure both the patient and the accepting facility are fully engaged in the warm handoff process.
- Assist patients in setting up and sustaining self-help groups, as well as means of locating and joining existing groups throughout the community.
- Utilize and teach problem solving techniques with individuals and groups
- PRSS will share with patients their skills, strengths, supports, and resources they utilized in their own recovery and demonstrate how they have directed their own recovery plan.
- Complete intake assessment and Screening, Brief Intervention & Referral to Treatment (SBIRT) screenings.
- Assess Medication Assisted Treatment (MAT) options for patients and induction of buprenorphine prior to discharge. Confirm transportation to intake appointments and confirm patient arrival and engagement
- PRSS will have meeting and warm hand-off with the team member from the receiving treatment center at the hospital
- Collaborate with the OUD/SUD team to coordinate an appointment for buprenorphine, methadone, or naltrexone
- Responsible for scheduling the appropriate follow-up appointment and making a warm handoff with clear instructions.
- Ensure around-the-clock resources are available for all patients and families
- If transportation is not offered from accepting facility, PRSS will arrange transportation services.
- Ensuring a connection to affordable treatment options and recovery services. Engage with patients discharge.
- Collaborate with the interdisciplinary team to assess the patient's support systems and engage in family meetings regarding treatment decisions as needed
- Verify the patient's insurance status to confirm the proposed treatment plan is covered
- Seek out providers who can offer services on an affordable sliding fee scale
- Coordinate with the hospital's financial counselors who can help patients enroll in Medicaid during their stay.
- PRSS to remain in contact with discharged patients through follow-up phone calls and contacts 24 hours post discharge and 1 week later.
- Maintain professional relationships with patients after discharge through after-care recovery groups organized by the program
- Maintain and stay connected within a referral network for other health and human services that would provide for this specific population of patients
- Offer enrollment with grant-directed follow up studies.
- Professional Development
- Attends in-service training as scheduled by supervisor or manager.
- Attend continued education on EBP regarding the latest therapeutic and medication treatments.
- Minimum two years of sustained sobriety who identifies as being in recovery from a substance use disorder and is willing to share details of his or her personal recovery experience.
- Basic PC skills Excellent communication skills
- CPR & Crisis Prevention Intervention.
High school diploma or equivalent.
