Your Role Will Include
- Performing thorough patient evaluations, ordering necessary diagnostic tests, and developing customized treatment plans
- Diagnosing and addressing acute, chronic, or episodic conditions collaboratively with physicians or independently (depending on credentials)
- Providing education to patients and their families on wellness, preventive care, and disease management
- Prescribing medication and therapies in accordance with your scope of practice
- Engaging in patient follow-up and chronic care management initiatives
- Partnering across interdisciplinary teams to ensure cohesive and comprehensive patient care
- Accurately documenting in electronic medical records (EMR)
- Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
- Completion of an accredited Nurse Practitioner program
- Active Nurse Practitioner licensure in the state of New Jersey
- National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or an equivalent specialty
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or the ability to acquire them within 90 days)
- A strong clinical expertise complemented by a caring approach
- Exceptional communication skills with patients, families, and clinical teams

- Familiarity with EMR systems and comfort with telehealth technologies
