Nurse Practitioner

HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with Managed Care partnered SNF in Fairview Manor, PA.

Daily Responsibilities for Nurse Practitioner:
  • Conduct patient facing comprehensive assessments between 4 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions
  • Conduct advanced illness and advanced care planning conversations for 4-5 patients/day
  • Mon - Fri 8am-5pm
  • 3-6 month conversion to FTE

Required Qualifications for Nurse Practitioner:
  • Graduate of an accredited Master of Science Nursing program
  • NY Board Certified Family, Adult, Gerontology or Adult-gerontology Acute Care NP
  • DEA Prescriptive Authority
  • Covid Vaccination
  • 1+ years of clinical experience in NP Roles

Preferred Qualifications for Nurse Practitioner:
  • Doctor of Nursing Practice
  • Meeting the medical needs of patients with complex behavioral, social and/or functional needs
  • Understanding of Advanced Illness and end of life discussions

Pay Details: $75 - $80 an hour
