Nurse Practitioner HealthCare Support is actively seeking a Nurse Practitioner to fill an opening with Managed Care partnered SNF in Fairview Manor, PA. Daily Responsibilities for Nurse Practitioner: Conduct patient facing comprehensive assessments between 4 SNF facilities to prevent unnecessary hospital readmissions for acute and chronic conditions Conduct advanced illness and advanced care planning conversations for 4-5 patients/day Mon - Fri 8am-5pm 3-6 month conversion to FTE Required Qualifications for Nurse Practitioner: Graduate of an accredited Master of Science Nursing program NY Board Certified Family, Adult, Gerontology or Adult-gerontology Acute Care NP DEA Prescriptive Authority Covid Vaccination 1+ years of clinical experience in NP Roles Preferred Qualifications for Nurse Practitioner: Doctor of Nursing Practice Meeting the medical needs of patients with complex behavioral, social and/or functional needs Understanding of Advanced Illness and end of life discussions Benefits for Nurse Practitioner: Immediate enrollment in Health Insurance Dental Insurance Life Insurance Employee Assistance Program (EAP) Access to Investment Accounts Career and educational tools within our Ingenovis ACT (Advocacy) Program Pay Details: $75 - $80 an hourInterested in being considered?
