Description
- Assess patients in both inpatient and outpatient settings and develop personalized treatment strategies
- Implement therapeutic interventions aimed at enhancing mobility, strength, balance, and overall function
- Work closely with nurses, physicians, case managers, and rehab specialists to ensure the best outcomes for patients
- Guide and educate patients and families on their therapeutic journey, discharge plans, and appropriate home exercises
- Maintain thorough documentation of evaluations, treatments, and patient progress while adhering to facility and legal standards
- Engage in care team meetings and interdisciplinary discussions to ensure cohesive patient care
- Doctorate in Physical Therapy (DPT) or an equivalent qualification from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred
- Additional certifications in specialized areas (e.g., neurological, geriatric) enhance your candidacy but are not mandatory
- Preferred previous experience in a similar role
- Ability to treat patients with diverse medical and mobility challenges
