Occupational Therapist needed asap for 13 weeksREQUIRED SKILLS: Active Drivers License, Auto-Insurance, Reliable Vehicle, and clean DMV Report - RequiredGeneral OT - no specific specialty (need to see pediatric and general population)Mostly Outpatient w/ Acute CareOrtho, Neuro, Peds Experience - RequiredBLS (AHA)ALL SCHEDULING REQUESTS MUST BE MADE AT THE TIME OF SUBMISSION (Facility will NOT approve scheduling requests at/after time of offer / Please obtain this information prior to submittal / Requests after offer can be cause for rescinded offer)Dress Code: Uniform or dress/casual clothes no jeans

Skills Checklist, current to 1 year, pertinent to the specialty
2 Supervisory Reference Forms, taken within the last two years
Licensure (MI LARA) & Certifications (BLS - AHA)
Candidates' Permanent Address (Facility will not consider local candidates - 50 mile radius restriction)
