Home Health Clinical Manager (RN)

(Also known as Director of Nursing or Director of Clinical Services - Home Health)

  • Location/Type: Naperville, IL • Onsite (serving Bolingbrook, Frankfort, Yorkville, Romeoville, and nearby suburbs)
  • Pay: $95,000-$113,000 base + mileage reimbursement + full benefits
  • Schedule: Full-time, Monday-Friday
As a Home Health Clinical Manager (RN), you'll guide a skilled team delivering high-quality, Medicare-certified home health care throughout the western suburbs. You'll oversee nursing, therapy, and aide staff-ensuring OASIS accuracy, compliance readiness, and compassionate patient care. Supported by strong intake, QA, and field teams, you'll lead with autonomy, not bureaucracy.

Ideal for RN leaders with home health or skilled nursing (SNF) experience ready to move into a full branch clinical leadership role.
What you'll do
  • Oversee nursing, therapy, and aide teams to ensure exceptional patient outcomes
  • Review OASIS documentation, QAPI data, and audits to keep the branch survey-ready
  • Mentor and develop field clinicians through coaching and hands-on leadership
  • Collaborate with the Executive Director on outcomes, rehospitalizations, and staffing metrics
  • Partner with referral sources and community contacts to strengthen transitions of care
  • Participate in interdisciplinary (IDG, QAPI) meetings and lead continuous improvement efforts
  • Step into the field only when needed-your focus is on guiding care, not endless visits
Must-haves
  • Active Illinois RN license
  • 2+ years of home health or skilled nursing (SNF) experience
  • 1+ year of supervisory or charge nurse experience
  • Strong understanding of Medicare guidelines, OASIS, and CHAP or equivalent accreditation standards
  • Collaborative leadership style with the ability to motivate and grow a team
