Primaryresponsibilities include providing direct and individualized care to patients including: Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives. Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions. Participates in case reviews to evaluate case management and progress. Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan. Assists with discharge planning and processes. Identifies appropriate resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan. Supervises and/or trains new staff, students, and interns. Performs Medicaid screenings as indicated for NHP. Assists patients with Safety Net applications when needed. QualificationsMinimum Qualifications: Education: Masters degree or above in related field. Eligible for clinical licensure in NV. Required Skills: Requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment and take appropriate action. License and Certifications:NV license required (LCSW, LMFT, CPC, Psychologist or licensed intern in any of those areas) .AHA Basic Life Support (BLS) upon hire, and Handle with Care required within 30 days of employment. Experience:A minimum of one (1) to two (2) years working in similar setting preferred.
