ResponsibilitiesConduct clinical case reviews, share tools, information, and guidelines as they relate to quality of careRepresent TeamHealth in engaging, establishing, and maintaining relationships within the communityCollaborate with leadership to review care coordination initiativesDevelop intervention plans, and share ideas about network and risk management issuesWork alongside pharmacists on medication managementImplement an individualized patient care plansTimely response in chart completions and pharmacy recommendationsProvide oversight and mentorship to our clinicians in the greater Albany, New York, areaExperience as a medical director in a skilled nursing facility preferred. However, clinicians who are board certified and experienced in the following disciplines are encouraged to apply: family (FM), primary care, internal (IM), emergency, (EM), adult, post-acute, home health, hospice, palliative, urgent care, and hospital medicine (HM). Compensation is fee for service (FFS). Expected compensation is estimated salary range of $310,000 to $345,000 with no cap on productivity income potential.
