Why LPNs Choose Home Matters Guaranteed 10 hours per week to start Clear path to full-time as the agency continues to grow Flexible scheduling with autonomy over your day $28.50/hour (paid hourly, not per visit) Supportive, accessible leadership that respects nursing judgment Variety in your work—no facilities, no repetitive shifts Purpose-Driven Care and Nurse-Guided Care philosophy What You’ll Be Doing Conduct in-home nursing assessments Create and update individualized care plans Delegate and supervise caregiver tasks per the Colorado Nurse Practice Act Train and coach caregivers to ensure safety, confidence, and compliance Complete supervisory visits and 30/60/90-day reviews Communicate with families, caregivers, hospice partners, and leadership Serve as a clinical resource when questions or changes arise Step in to support care continuity when needed This is a hands-on, people-first role—not a desk job and not a facility role. Where You’ll Work Office: Brighton, CO Service Area: Brighton, Broomfield, Thornton, Northglenn, Commerce City, Westminster, Erie, Frederick, and Lyons Travel to client homes, supervisory visits, and occasional office check-ins What We’re Looking For Active, unencumbered Colorado LPN license 1+ year of home care, home health, or similar experience Hospice and/or dementia experience preferred Comfortable with delegation, training, and caregiver supervision Strong communication and documentation skills Valid driver’s license and reliable transportation Comfortable traveling throughout North Denver Basic EMR and computer skills
