With unique services designed for each state, we provide trauma-informed care and community-based youth programs in Connecticut, comprehensive family support and residential services for at-risk youth in New York, and targeted mental health services and crisis intervention in Rhode Island and so much more. Across all three states, our goal is to empower individuals and strengthen families. Familias Unidas Program Coordinator will provide case management services to families in Orange County. The Program Coordinator has a pivotal role in supporting families to achieve goals, address needs that lead to DSS involvement and support safety and well-being children and families. To engage and effectively work with families within their natural environment, the Program Coordinator provides services in the family’s home and community. This is an integral role responsible for supporting families, collaborating with community providers, connecting family to services, and ensuring safety and well-being. Familias Unidas™ is a multilevel family-based intervention designed to prevent substance use and sexual risk behavior in Hispanic adolescents. The program is influenced by culturally specific models developed for Hispanic populations in the United States, and is delivered primarily through multi-parent groups, which aim to develop effective parenting skills. The multi-parent groups, led by a trained facilitator, meet for 8 weekly two-hour group sessions and 4 one-hour family visits during the 12 weeks. Each group has 12 to 15 parents, with at least one parent from each participating family. Group sessions are conducted by two trained facilitators and include problem posing and learning exercises. Group discussions aim to increase parents’ understanding of their role in protecting their adolescents from harmful and risky behaviors and to facilitate parental investment. Responsibilities Include: Complete intake screenings of new referrals in accordance to NAFI NY and Familias Unidas standards Complete pre and post assessments according to NAFI NY and Familias Unidas standards Deliver Familias Unidas curriculum to Latino/Hispanic youth, adolescents and families who reside in Orange County Facilitate group sessions in-person for prospective participants in conjunction with a co-facilitator Facilitate in-home family sessions to program participants Provide support and guidance to families to achieve service plan and treatment plan goals. Provide linkages to community services to the family to ensure they receive needed support Conduct minimum required visits with families each month as per NAFI NY, Familias Unidas, DSS, and OCFS requirements Collaborate with Department of Social Services on family’s needs and services being provided Assist with crisis management, intervention, and de-escalation when needed Document progress notes, reports, treatment/service plan that demonstrate the case management support provided to families including success and challenges of the family’s progress toward treatment goals Report both written and orally to stakeholders and courts on the progress and barriers the family is experiencing Attend professional development and/or trainings relevant to increasing knowledge, skills and attitude required to deliver program effectively and with fidelity. Participate in clinical weekly supervision meetings as directed by supervisor. Maintain all required program paperwork and records, including recordings of sessions as required. Participate in all general staff meetings as required.
