Position HighlightsDepartment: QualityHours: 40.00 per weekPosition SummaryThe Stroke Program Coordinator is responsible for planning, implementing and coordinating stroke services and activities associated with Middlesex Hospital. Responsible for establishing and monitoring clinical performance criteria for assuring compliance with our Advanced Stroke Center regulatory requirements.Essential Duties & ResponsibilitiesUnder limited supervision or directionTogether with the Advanced Stroke Program Physician Leadership, coordinates and facilitates the stroke program at Middlesex Hospital.In collaboration with the program leaders, monitors the performance of the advanced primary stroke program’s goals in order to achieve the mission, objectives and programmatic goals.Primary lead for Joint Commission preparation for recertification of our advanced stroke program.In collaboration with the program leaders, designing, implementing, and evaluating care, treatment, and services.In collaboration with the program leaders, facilitates the Stroke Committee to provide best practice care without variation, thereby maximizing best outcomes for patients.Together with the Advanced Primary Stroke Program Physician leadership and committee, contributes to the continuous review of established stroke pathways and maintaining evidenced based practice.Together with the leadership team, advises administration on the needs of the Advanced Primary Stroke Committee.Oversees compliance and facilitates on-going staff educational opportunities for all levels and departments interacting with the stroke population.Collects and analyses on-going data regarding outcomes of the stroke programSubmits required data to the joint commission and other regulatory agencies as requested.Maintains clinical knowledge and skills for the stroke patient population.Resource to the hospital for care of the stroke patient regardless of admitting diagnosis of the patient.Contributes to other department Quality Improvement, Patient Safety, Regulatory work as required and directed by the Director (i.e. trauma registry, regulatory surveys, root cause analysis, leadership rounding).Minimum QualificationsBSN or related clinical healthcare degreeCT RN licenseMasters Degree required (preferred in Nursing): Can be currently enrolled in a Masters program with expected graduation within two yearsDemonstrates self-motivation, independent and goal orientated professionalMust demonstrate knowledge and understanding of cerebrovascular physiology, pathophysiology and clinical processes for careDemonstrates excellent assessment, organizational and communication skillsDemonstrates excellent critical thinking analysis and assessment skillsDemonstrates excellent interpersonal, communication and problem solving skills and ability to lead and motivate multidisciplinary teamsPreferred QualificationsExperience in Quality ImprovementTwo years experience in healthcare managementComprehensive Benefits OfferedCompetitive and affordable benefits packageShift DifferentialsContinuing Education assistanceTuition reimbursementStudent Loan relief through FiduciusQuick commute access from I-84, Route 9 and surrounding areasAbout Middlesex HealthThe Smarter Choice for your CareerCome join one of Connecticut’s Top Workplaces, and a Magnet designated organization At Middlesex Health, we have a unique combination of award-winning talent, world-class technology, and patient-first care that's making health care better.
