AVH+ is a locally owned company with owners actively participating in the day-to-day operations of the agency performing as part of the team. We are fully staffed and looking to expand our team in order to support and anticipate our growing patient census's needs.
Position Overview:
We are seeking a Licensed Vocational Nurse (LVN) to join our team as a Hospice LVN Intake/Office Coordinator. In this role, you will play a vital part in facilitating new patient admissions and managing office coordination to ensure smooth operations. The ideal candidate will bring clinical expertise, administrative skills, and a compassionate approach to patient care, with Spanish-speaking abilities preferred.
Responsibilities:
- Handle patient referrals and complete the intake process, ensuring all necessary documentation and patient eligibility requirements are met
- Communicate with families, healthcare providers, and other stakeholders to provide information and coordinate patient transitions into hospice care
- Conduct patient assessments as required to support clinical needs during the intake process
- Explain hospice benefits and services to patients and families as needed
- Maintain accurate, HIPAA-compliant records in electronic health records and other intake systems
- Support the office team with scheduling, call management, and general office duties
- Collaborate with interdisciplinary team members to ensure timely and effective patient care
- Maintain confidentiality and uphold high standards of patient and family communication
- Other job duties and special projects as assigned.
Qualifications:
- Current LVN license in the state of California (required)
- Experience in hospice, home health, or similar healthcare setting
- Bilingual in Spanish (preferred) to communicate effectively with Spanish-speaking patients and families
- Strong organizational and multitasking skills with attention to detail
- Proficient in medical terminology, electronic health records, and office software (HospiceMD)
- Compassionate approach to end-of-life care and ability to provide emotional support to patients and families
- Current California driver's license and proof of auto insurance
