We are currently seeking a highly organized and dedicated Clinical Intake Coordinator to join our team. This role is crucial in managing the intake process for new patients, ensuring a smooth transition to our care services.


Position Summary:

The Clinical Intake Coordinator is responsible for managing all incoming referrals, processing patient admissions, and coordinating care with our clinical team. The ideal candidate will have prior experience as a Licensed Practical Nurse (LPN) and Health Unit Coordinator (HUC), providing them with the clinical insight and administrative skills necessary to excel in this role.


Schedule:

This position offers two shift options: 8:00 AM - 5:00 PM or 10:00 AM - 7:00 PM, with a weekend rotation required.


Key Responsibilities:

  • Handle all incoming patient referrals and inquiries, ensuring timely and accurate data entry into the system.
  • Collaborate with clinical and administrative teams to schedule and coordinate initial patient assessments.
  • Verify insurance eligibility and ensure that all necessary documentation is complete and compliant.
  • Communicate effectively with referral sources, patients, and families to explain services and answer questions.
  • Maintain a working knowledge of home care regulations and patient care processes.
  • Monitor the status of ongoing referrals and admissions, updating records and staff as needed.
  • Provide administrative support to clinical staff to ensure smooth day-to-day operations.

Qualifications:

  • Active and unrestricted Licensed Practical Nurse (LPN) license required.
  • Prior experience as a Health Unit Coordinator (HUC) is highly preferred.
  • Strong knowledge of medical terminology, patient care procedures, and home care regulations.
  • Excellent organizational skills and attention to detail.
  • Ability to manage multiple tasks and work in a fast-paced environment.
  • Proficient with electronic medical records (EMR) systems and Microsoft Office Suite.
  • Strong interpersonal and communication skills, both verbal and written.
  • Ability to work collaboratively with a diverse team of healthcare professionals.
