ACRC is seeking Service Coordinators

We are hiring in the following units

- Residential unit

The Service Coordinator provides case management and advocacy for persons with developmental disabilities. This position is currently located in the Sacramento office on a HYBRID schedule. Typical duties include assisting clients and their families in acquiring and maintaining assessed supports and services including transportation, vocational training, day programming, education, mental health services, medical care, and independent living training; Responsibilities include participation in the development of the Individual Program Plan (IPP); monitoring the IPP and assisting with revisions as necessary; assisting clients and their families in acquiring and maintaining assessed supports and services; serving as an advocate for the client with community agencies; consulting with and assisting vendors with certifications, applications, and referrals; participation in the development of Individual Program Plans for clients; monitoring these plans and revising them as necessary; completing all required forms, documentation, and reports in accordance with regulations and ACRC policies and procedures; rotating officer-of-the-day duties with other Service Coordinators; assisting co-workers with special projects, unique problems, vacation, and sick relief; and providing emergency on-call services when required.

The Early Intervention Service Coordinator will have cases anywhere in the Sacramento area and will provide case management and advocacy for children from birth to three years of age with a developmental delay, established risk condition, or who are at high risk of having a developmental disability. Responsibilities include conducting psychosocial and developmental screening assessments in the home environment; developing individual family service plans (IFSP) with families and collaborative agencies; coordinating services for infants and their families; determining ongoing eligibility of clients at age 3;

The Children Service Coordinator may have cases anywhere in the greater Sacramento area. This position serves children with developmental disabilities ages 3-18.

The Adult Service Coordinator serves adults with developmental disabilities over the age of 18.

The Residential Service Coordinator serves clients residing in licensed residential facilities.

REQUIREMENTS

Service Coordinators must have a Bachelor's degree or a Master's degree in Social work or a related field. Must have at least one year of professional experience in social work or a related field is preferred. Must demonstrate excellent written, verbal, and interpersonal communication skills.
