TITLE: Psychiatric Rehabilitation Coordinator

GENERAL SUMMARY:

The rehabilitation coordinator, under the program manager and/or director, and the rehabilitation specialist will provide direct services to consumers in order to help them develop and/or restore their independent living skills. The rehabilitation coordinator will record reports of all activities and/or interventions in the consumer's medical record reports and provide feedback/reports to the appropriate professional staff. The rehabilitation coordinator is responsible for assisting in the development or implementation of the consumer IRP.

PRINCIPAL RESPONSIBILITIES AND DUTIES:

 

1. Plan, organize and conduct rehabilitation assessment, evaluation, and intervention for a client on caseload, including orienting as well as ensuring the client's, family and/or legal guardian when permitted in participating and developing the client's individual rehabilitation plan.

2. Teach clients independent living skills, including identifying and addressing gaps in service provision as well as teaching client community integration skills.

3. Provide supportive counseling to severely emotionally distressed clients on caseload, as well as act as Client's Advocate when applicable.

4. Review and coordinate the collection of relevant information on referred client's background, social history, and past treatment interventions in order to develop rehabilitation plans with and for the clients. Communicating information regarding the progress of the client to referral source i.e therapist and/or psychiatrist.

5. Develop and implement individualized rehabilitation plans for assigned clients to meet individual needs, interests, abilities, and share information with clients on how to access community resources to meet their individual needs.

6. Collaborate bi-weekly with the therapist of each client on your caseload and document this collaboration in the monthly summary.

7. Attend mandatory monthly staff meetings.

8. Evaluate, monitor, and review individualized rehabilitation plans and assess individual progress in the treatment and make recommendations for modification, changes, or termination of the treatment.

9. Maintain records as required by established laws, policies, and administrative regulations including COMAR regulations.

10. Facilitate transition, discharge, termination, admission, and clinical supervision as scheduled in order to promote client services and follow-up care.

11. May act as a consultant to outside providers, school personnel, and medical staff on the individual needs of the client's caseload.

12. Consult with the program manager and other outside resources to coordinate services to clients outside of the KHS.

 

KNOWLEDGE, SKILLS, AND ABILITIES REQUIRED:
 

· Effective communication and interpersonal skills to develop positive relationships with community organizations.

· Ability and desire to work with and advocate for all populations, including the disabled.

· Must possess a current MD State-issued Driver’s License and have a current driving record without points on it.


WORKING CONDITIONS:

1. Normal activity-center environment, where there is no physical discomfort due to temperature, dust or noise. Depending on position, may have some exposure to cigarette smoke in an area designated for client smoking.

2. Offsite visits in clients' home environment.

3. Depending on a specific assignment, work requires travel between programs and services.

REPORTING RELATIONSHIPS:

1. Must be supervised by the Rehabilitation Specialist, Program Manager, and/or Program Director.

MINIMUM QUALIFICATIONS

· High School. preferred BA degree in human services, art, or social services with at least one (1) year of experience working with clients with severe emotional or psychiatric problems is preferred

· Or, a high school diploma High School with excellent writing skills, and some course work in social services or human services with at least 3 years of experience in providing rehabilitation services to clients with severe emotional problems.

· Candidate must show documented experience in planning rehabilitation recreational or rehabilitation activities for clients with a physical, emotional, or medical disability.
