Position Overview The Group Coordinator is responsible for leading groups within the following Office of People with Developmental Disabilities (OPWDD) Home and Community-Based Services (HCBS) Waiver programs: Day Habilitation. This includes but is not limited to overseeing the days schedule for their assigned group, positive role modeling, promoting the well-being and independence of people supported, and ensuring compliance with regulations and agency policies.
  • Performance Management: Provide meaningful guidance to Direct Support Professionals (DSPs) including ensuring evaluation completion and support as needed. Directly oversee the DSP assigned to group.
  • Case Management: Oversee the assigned group of people supported in Day Habilitation. This includes ensuring the health and safety of those supported and overall program satisfaction.
  • Service Planning and Coordination: Attend and facilitate person centered meetings to collaboratively develop Staff Action Plans for those supported in Day Habilitation. Coordinate and/or facilitate support meetings (i.e., Interdisciplinary Team Meetings), as needed. Attend all necessary meetings (i.e. LifePlan Meetings), as needed.
  • Relationship Development: Develop meaningful relationships with people supported, advocates, and family members by being in contact at minimum on a daily basis this includes but is not limited to a variety of different communication methods such as utilizing a communication log, daily phone calls, daily texts, daily in person conversations, etc. Maintain daily communication with support teams as needed.
  • Comprehensive Support: Offer diverse forms of support, including but not limited to personal care, money management, and educational support, while promoting individual choice and teachable moments.
  • Schedule and Community Outings: Coordinate and implement a daily schedule for assigned group. Accompany and transport people supported to community outings and work sites using various modes of transportation (i.e., public, personal, or company vehicles) as required.
  • Documentation: Maintain daily documentation of services in the Employee Health Record (EHR) to communicate progress, needs, behavioral incidents, and management concerns. Oversee completion of daily service delivery and monthly summary documentation alongside the DSP(s) assigned to the group.
  • Communication with Supervisor: Ensure ongoing communication with your supervisor during shifts.
  • Service/Treatment Plans: Adhere to the safeguards, Levels of Supervision, and support response methods outlined in service and treatment plans.
  • Financial Recordkeeping: Oversee and record expenditures, submit receipts, and handle petty cash for the assigned group.
  • Flexibility: Be prepared to perform other duties as assigned by supervisors and respond to changes in job responsibilities.
  • Technology: Stay updated with technological usage, including emails, Paycom, eVero, timekeeping, and password management
  • Engagement: Oversee overall engagement of the people supported and DSPs within assigned group. Participate in meetings and discussions involving the people supported.
  • Emergency Response: Assist with implementing systems for responding to crises and/or emergencies for people supported.
  • Advocacy: Advocate for the needs and rights of people supported, ensuring they are receiving adequate services and support.
  • Reporting: Ensure proper reporting, investigation, and review of all incidents relative to OPWDD Part 624 regulations and the New York State Justice Center.
  • Professional Development: Complete all mandatory and assigned trainings including specific OPWDD required trainings as needed. Take initiative in asking for support in areas needed. Support the administrative team with identifying training needs for DSPs.
  • All other duties deemed necessary by the supervisor.

  • Professionalism: Strong commitment to meeting the needs of participants, designees, families, and advocates. Takes personal responsibility for resolving service problems.
Qualifications
  • Age Requirement: Minimum age of 18.
  • Experience:
    • 1+ year(s) working in OPWDD HCBS Waiver programs including but not limited to Day Habilitation, required.
    • 1+ years in a supervisory/leadership role, preferred.
  • Educational Qualifications: High School Diploma, required.
  • Driver's License Requirement: Valid Driver’s License, required.
  • PCCS DSP Career Ladder Program:
    • Enrollment in the PCCS DSP Career Ladder Program in the next available cohort
    • Maintain good standing within the program (i.e. completion of trainings, attending meetings, etc.)
