What You'll Do:

Evaluate and implement personalized care plans, ensuring high-quality patient care in the comfort of their home
Perform skilled nursing assessments and adjust treatment plans as patient needs evolve
Provide hands-on care including medication administration, wound care, and therapeutic treatments
Monitor patient health status and promptly communicate any changes to physicians
Document all care activities thoroughly using our user-friendly electronic health records system
Educate patients and families on health management, medications, and care procedures
Coordinate with our healthcare team to ensure comprehensive patient support
Set achievable health goals with patients and develop effective discharge plans
Use your nursing expertise to make real-time decisions that improve patient outcomes

What You Need:

Current BLS certification
Up-to-date health records (PPD/Chest X-Ray)
1+ year nursing experience preferred
Passion for patient-centered care
