What You'll Do:

  • Evaluate and implement personalized care plans, ensuring high-quality patient care in the comfort of their home

  • Perform skilled nursing assessments and adjust treatment plans as patient needs evolve

  • Provide hands-on care including medication administration, wound care, and therapeutic treatments

  • Monitor patient health status and promptly communicate any changes to physicians

  • Document all care activities thoroughly using our user-friendly electronic health records system

  • Educate patients and families on health management, medications, and care procedures

  • Coordinate with our healthcare team to ensure comprehensive patient support

  • Set achievable health goals with patients and develop effective discharge plans

  • Use your nursing expertise to make real-time decisions that improve patient outcomes

What You Need:

  • Current BLS certification

  • Up-to-date health records (PPD/Chest X-Ray)

  • 1+ year nursing experience preferred

  • Passion for patient-centered care
