We are seeking compassionate and skilled Registered Nurses to join our dynamic Hospital at Home team in Temple, Texas. As a Hospital at Home Registered Nurse, you will have the unique opportunity to provide personalized care, assess clinical needs, foster a healing environment, and support patients and their families through education and emotional support.

*Key Responsibilities*
* Provide high-quality acute care services to patients enrolled in the Hospital at Home program.
* Conduct comprehensive assessments and develop individualized care plans tailored to each patient’s needs.
* Spend additional time at the bedside to assess clinical and environmental needs, ensuring a supportive healing environment.
* Educate and provide ongoing support to patients and their families, ensuring they have the resources and understanding necessary to manage their care at home.
* Collaborate closely with interdisciplinary teams to coordinate care and optimize patient outcomes.

*Job Summary*
As a Registered Nurse, you will oversee patient care provided by the healthcare team using a professional practice model. Your responsibilities will include patient assessment, recognizing health issues, creating care plans, and assessing patient responses.

*What You'll Do*
* Conduct detailed clinical assessments, prioritize patient and family needs, and create, implement, and evaluate care plans to achieve desired outcomes.
* Build therapeutic relationships by managing discomfort and maintaining professionalism in your interactions with patients and families.
* Create care plans that reflect patient values to aid in resolving health issues.
* Educate patients, families, nurses, and community members to empower them with knowledge for better healthcare choices.
* Collaborate with colleagues and community resources to develop care plans, delegate tasks, and follow up promptly.
* Employ problem-solving strategies to address challenges faced by patients, families, and staff, always prioritizing safety, effectiveness, and efficiency.
* Achieve goals through teamwork, recognize contributions, and commit to personal and professional growth.

* Deep understanding of nursing and patient care standards, procedures, and regulations.
* Knowledge of nursing laws, rules, standards, and guidelines specific to your area.
* Proficiency in medical terminology, including health management and common medications.

* Exceptional communication skills for clear verbal and written expression.
* Strong interpersonal skills to facilitate effective interactions with diverse individuals.
* Excellent problem-solving capabilities and critical thinking skills.
* Basic computer skills, including familiarity with Microsoft Office, electronic medical documentation, and scheduling systems.

*Qualifications*
* Graduate of an accredited nursing program.
* Current and valid nursing license.
* BLS certification required (or must obtain within 30 days of starting in this role).

Pay: $32.00 - $53.00 per hour
