We are currently offering a $5000 sign on bonus, competitive salary and benefit package.

*Responsibilities:*
* Manages primary caseload of Hospice patients per physician's instructions and in compliance with hospice regulations and Medicare guidelines
* Implements and updates care plans to address patient's nursing needs.
* Educates and supports patients, family, and staff on nursing care and patient needs.
* Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements.
* Supervises Licensed Practical Nurses and Hospice Aides when needed
* Some on-call responsibilities on a rotating basis with additional financial compensation

*Requirements:*
* 2+ years’ experience in hospice or palliative care but willing to train the right person.
* Ability to collaborate within an interdisciplinary setting.
* Willingness to learn about hospice philosophy and principles of death/dying.
* Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements

*Skills:*
* Exceptional communication and interpersonal skills
* Robust critical thinking and problem-solving abilities
* Capable of working independently and collaboratively in a multidisciplinary team
* Proficient in electronic medical record systems and relevant software applications
