Essential Functions:
- Manages primary caseload of Hospice patients per physician's instructions and in compliance with regulations.
- Implements and updates care plans to address patient's nursing needs.
- Educates and supports patients, family, and staff on nursing care and patient needs.
- Collaborates with the Interdisciplinary Team (IDT) to meet all patient and family care requirements.
- Supervises Licensed Practical Nurses and Hospice Aides.
- Certified Hospice and Palliative Nurse (CHPN), preferred.
- 2+ years' experience in hospice or palliative care but willing to train the right person.
- Ability to collaborate within an interdisciplinary setting.
- Knowledge and understanding of hospice philosophy and principles of death/dying.
- Valid driver's license and reliable, insured transportation in accordance with New Jersey state requirements.
