Primary Care Nurse

We are seeking an experienced and dedicated Primary Care Nurse to join a prominent healthcare facility in the heart of the city. The Primary Care Nurse will report to the Nurse Manager of the Primary Care Unit. The position involves close collaboration with physicians, advanced practice providers, and other healthcare professionals to ensure comprehensive patient care and effective management of primary care services.
The Primary Care Unit is a busy and supportive environment where nurses provide care to a diverse patient population. The unit focuses on managing chronic conditions, preventive care, and overall patient health. The team is dedicated to fostering a patient-centered approach, providing education and support to patients, and collaborating closely to ensure the best possible outcomes.
In this role, the Primary Care Nurse will conduct comprehensive assessments of patients’ health, develop and implement individualized care plans, administer medications and treatments as prescribed, and provide patient education on health management and preventive care. You will also collaborate with the healthcare team to coordinate patient care, monitor and document patient progress, and address any concerns or changes in patient conditions. Additionally, you will maintain a safe and effective care environment, ensuring compliance with healthcare regulations and standards.
The Primary Care Nurse will hold a Registered Nurse license and strong assessment, critical thinking, and communication skills are essential. Candidates with experience in managing chronic diseases, performing patient education, and working within a collaborative healthcare team will earn a quick interview.
