About the Role

CINQCARE, a provider-led healthcare organization, is seeking an experienced Nurse Practitioner or Physician Assistant to join our primary care team. This position offers a competitive salary and benefits package.

Responsibilities
  • Patient Assessments: Conduct thorough home visits to identify patients' needs, develop personalized care plans, and collaborate with primary care physicians to ensure seamless coordination.
  • Comprehensive Care: Provide accurate diagnoses, prescribe medications, and educate patients on disease management, medication adherence, and healthy lifestyle choices.
  • Collaboration and Communication: Work effectively with families, caregivers, and other healthcare professionals to deliver patient-centered care and address complex medical issues.
  • Data-Driven Decision Making: Utilize data and analytics to inform care decisions, drive quality improvement initiatives, and enhance patient outcomes.
Requirements
  • Education: Master's degree in Nursing (MSN) or Physician Assistant Studies (PA-C), APRN certification, and current DEA licensure.
  • Experience: Minimum 2 years of clinical experience as a Nurse Practitioner or Physician Assistant in primary care or home care settings, with expertise in chronic disease management and medication options.
  • Skills: Excellent communication and interpersonal skills, ability to work independently, and proficiency in electronic health records systems.
