The Youth Housing Case Manager is responsible for creating and implementing case plans for the youth in our emergency shelter program. The Case manager works with youth, their families, other Cocoon House programs and community resources to assist the youth in getting their needs met and supporting reunification when possible. The Case Managers play a vital role un engaging and support youth on a daily basis and helping them reach their fullest potential. Cocoon House is committed to continually growing in our equity, diversity, and inclusion journey, and candidates with lived experience or who identify as BIPOC and/or LGBTQ+ are strongly encouraged to apply We will also provide reasonable accommodation to qualified persons with disabilities upon request.  Youth Housing Case Manager Duties & ResponsibilitiesFull-Time (Tuesday-Saturday)| On-site in Monroe Starting Wages: $22-24/hr depending on shift | +$1.50/hr pay differential for multi-lingual skills with additional paperworkMust have weekend availability Maintain a high level of supervision of youth in care, ensure safety at all times and help ensure youth are following program guidelines and expectations. Fill out all logs as required for the shift worked (meds, logs, pass down, etc.). Assist with transporting youth to school, appointments or recreational activities as needed. Attend case plan meetings, all staff meetings, new employee orientation and RYC training. Assist with taking pride in program by helping to assure cleanliness of the facility and program. As appropriate, assist youth with doing these household tasks with you and demonstrate it as a learning opportunity.As needed, help with facilitating intakes and exits of youth while on shift and complete the required date/documentation needed.Be able to dispense medications, help fill prescriptions as needed.Develop realistic long-term or transitional housing plans with residents when required.Make appropriate referrals for medical, dental, mental health, and chemical dependency needs. Ensure that residents are following guidelines of program.
