Licensed Practical Nurse LPN

Responsible for assisting with patient care in the Adult Day Health Center. This includes care such as medical, psychological, social, and persona/hygiene. Performs assessments routinely and reports changes to RN, physicians and to the families. Act as liaison among physicians, case managers, and families. Obtains required documentation for individual charts to facilitate waiver services. In communication with other offices, uses MWMA daily, tracks required monthly documentation. Typically 32 hours per week (7:30 am - 4:00 pm Monday - Thursday), however, will require additional hours to cover when RN or other LPN is off.
Primary Duties:
l. Assessment of patients' needs and identification of potential problems.
2. Assessment of adjustment to the Adult Day Health program and implement changes as needed.
3. Provision of a safe, healthy, and supportive environment reporting to maintenance all broken or defective equipment. Reporting all spills, uncertainty of unknown chemicals or odors, and electrical problems to the safety director.
4. Acting as a liaison between patient and their physician communicates with families and outside agencies on a regular basis. Coordinate with RN to attend all required patient care meetings as needed.
5. Uses MWMA daily to obtain required waiver documentation. Follows up with case managers to obtain required documentation. Tracks dates for care plans and annual renewals for waiver services.
6. Review and update of MAR’s on a scheduled basis, and prn.
7. Maintains an appropriate ratio of staff to patients according to the state regulations and patient acuity.
8. Ensures weights and vital signs per month and as needed. Other staff writes documentation as well, but the LPN is responsible for all documentation completion returned on a day-to-day and monthly basis.
9. Maintain a working relationship with the RN Program Manager and assist with appointments for assessments of new individuals as needed.
10. Reporting of all new admissions and discharges. Maintaining an updated emergency sheet on all patients and checking with the front office coordinator that the emergency sheet is present.
11. Assessment of staff skill and identification of staff weaknesses and strengths. The need for additional training will be communicated to the Program Nurse.
12. Will mentor and lead all staff to perform to their highest ability. Provide direction and implementation of treatment to meet met therapeutic and rehabilitative goals.
13. Assist with coverage in Program Rooms as needed including assisting with feeding, hygiene and activities.
Experience with HCBW/SCL & MPW helpful. Experience with the IDD population Required. Must have current Kentucky Nursing License
