Job Description & Details:
To provide Physical Therapy to consumers in their home.
To contribute to the HHN Programme through chart notes and narrative based reporting.
To communicate with HHN’s Administrative Groups regarding the progress towards and the setting of consumer-outcomes agreed upon by our case management teams.
To believe and contribute to a future of healthcare in which all consumers have the ability to access services they need.
This job is a W-2 position, with freedom of scheduling. Harmony’s management structure always puts the therapists and consumers first. Our priority is matching the right therapists, with the right consumers, for the right services. Additionally, there are bonus and incentive programs within Harmony that revolve around procuring, administering, and scoring assessments provided by the company.
Caseloads are built with and for the therapist based on consumer fit, and comfortability with the family. HHN procures consumers based on their need, geographic location, and eligibility.
All HHN Professionals are involved and encouraged to participate in the matching process.
All responsibilities outside of chart notes, plan of care submittals, and the rendering of services, are completed by the front office (HHN’s Administrative Groups).
Our “hybrid office” model allows for optional coordination between HHN Professionals regarding setting goals, therapy tactics and strategies, and the exchange/checkout of HHN’s library of therapy materials and tools.
Harmony Administrative Groups:
HHN takes pride in reducing the stress and responsibility of its healthcare providers, especially with respect to administrative tasking.
Administrative groups are broken out into roles and responsibilities necessary to allow Harmony HQ to focus on paperwork, so that our practitioners can focus on people-work.
All therapy/medical documentation is completed via a web based Electronic Health Record (EHR) system accessed via personal computer, tablet or smartphone.
HHN’s Administrative Groups are responsible for all billing and coding matters, authorizations, intake and discharge paperwork.
HHN’s Administrative Groups are responsible for consumer notifications, communications, and scheduling outside the scope of services rendered by the matched HHN Professional.
HHN Professionals are only responsible for providing service, completing chart notes, and updating goals and outcomes for their respective consumers.
HHN hopes to provide all necessary tools and opportunities for HHN Professionals to make the most of their time spent with their consumers.
Harmony HQ:
Harmony HQ is located at 12500 E Iliff. Ave, unit 240, Aurora CO, 80014.
At Harmony HQ, HHN Professionals are encouraged to meet, participate in company-wide discussions, engage with new therapy tools and strategies, and collaborate with other HHN Professionals.
Work Schedules:
Harmony is currently hiring PTs to provide therapy in our consumers homes’ for all work schedules. Appointments are coordinated and scheduled by the HHN Professional in conjunction with the family, and in accordance with HHN’s consumer matching process. HHN’s Administrative Groups are always available and provide assistance throughout every step of the scheduling process, and all HHN Professionals are provided with workflows and other tools to help create the schedules they desire to work. Treatment plans/schedules are designed with ample consideration for work/life balance, appointment frequency, as well as engagement and effort required by the services rendered.
Flexibility is our goal, however HHN does have standard employment schedules regarding typical schedule, appointment frequency, and time of day preferences.
Late Part-Time (EPT) 0-10 hours per week
Scheduled from 12:00 pm until 5:00 pm
Early Part-Time (LPT) 0-10 hours per week
Scheduled from 8:00 am until 12:00 pm
Fixed Full Time (FiFT) 10-40 hours per week
Scheduled during normal business hours
Flex Full Time (FlFT) 10-40 hours per week
HHN Professionals may work weekends if they desire, based on their consumers’ availability and eligibility.
Compensation & Benefits:
Harmony is intent on being a preferred employer and as such, is willing to coordinate
compensation and benefits that are appropriate to each candidate. Compensation ranges shown below are subject to change and will be commensurate with discussed and agreed upon work experience, schedules and caseloads. Depending on the severity of the case, the individual compensation rates vary.
None are less than $50.00 per hour, and none are more than $120.00 per hour. The below rates reflect the average hourly rate for an average caseload at Harmony.
1+ Years of Experience; $60.00 per hour
5+ Years Of Experience; $75.00 per hour
10+ Years Of Experience; $90.00 per hour

Job Qualifications:
DPT - Applicants must possess a doctorate level degree in Physical Therapy (or equivalent degree) from a University or other Education Institution utilizing a Commission on Accreditation in Physical Therapy Education (CAPTE) accredited curriculum.
IMPORTANT: Depending on the applicants years of experience, education transcripts may be requested upon application submission.

YOE - Therapists Applying for this job must have worked professionally for 2+ years as or under a licensed PT.
DORA - All therapists are required to maintain an active registry and licensure by the Department of Regulatory Agencies’ (DORA).
NPI Number - All applicants must possess a Center for Medicaid and Medicare (CMS) National Provider Number (NPI) associated with their Social Security Number (SSN).
Drivers License - Applicants must possess a valid Colorado Driver's License.
IMPORTANT: Automobile Driver’s Insurance is required by Title 10 of the Colorado Revised Statutes (C.R.S.) Section 4 Part 6.
All PTs at HHN must have a working knowledge of physical, cognitive, learning and/or swallowing disorders.
Additionally, PTs must be aware of current trends in education standards for the assessment, diagnosis and treatment options available.
CPR - All HHN Professionals are required to be trained and certified as being capable of performing CPR on both infants, children and adults (HHN can assist in attaining this certification).
Job Duties:
HHN Professionals provide treatment to several consumer demographics that present with a variety of physical, language, cognitive, learning and/or swallowing disorders. All duties are subject to the specific needs of the consumers’ and their families’ in the home.
The duties listed below provide a representative sample of duties that we use with a large majority of consumers.
Assessment and diagnostic methods of physical, cognitive, and/or learning disorders.
Communicating regularly with the consumer and/or the consumer’s family or care providing team to provide awareness, education and a source of information for the family as the plan(s) of care requires.
Creating and implementing individual plans of care based on available assessment, diagnostic and screening results.
Keeping up to date with the development of technology applicable to current practice and treatment modalities being employed.
Collaborating with the consumer's care team including other professionals in the home and interagency representatives to ensure appropriate deliberation with all involved providers.
Documenting therapy and progress for each consumer by using appropriate goal setting and activity designs.
Developing and implementing educational opportunities with the consumers and/or their family to ensure their awareness of the kind and quality of the care being provided.
Planning, coordinating, and implementing a full range of consumer-focused treatment designed to optimize function (this may include designing and communicating.
therapy activities for the families to engage in between therapy appointments)
Documenting assessments and trial results for selecting Augmentative & Alternative Communication (AAC) interventions and technology, including speech-generating devices (SGDs).
Discussing eligibility and criteria for dismissal from, or admittance into, early intervention as well as other school-based or county-based services.
Observing and considering consumers’ (and family’s) cultural and linguistic environment to ensure sensitivity to the selection of appropriate assessment protocols.
Willing participation (and in some instances a bit of facilitation) in communications between consumers, advocates, other care providers and professionals that are interacting with the consumer.
