JOB DESCRIPTION

POSITION TITLE: Medical Group Home Licensed Practical Nurse LPN

REPORTS TO: Head Nurse Supervisor, Assistant Manager and Group Home Manager

QUALIFICATIONS: Must be at least 18 years of age. Have a high school diploma or equivalent. Active NJ LPN License. Valid NJ Drivers License in good standing. Medical Group Home experience is preferred.

STATEMENT OF POSITION PURPOSE: To assist four individuals with activities for daily and community living in accordance with the Agency & Division policies & to enhance the individual's capabilities and independence while advocating for their quality of life and rights as a person. To ensure the safety, health & well being of the individuals in their care through the accurate and consistent implementation of all plans.

RESPONSIBILITES INCLUDE BUT ARE NOT LIMITED TO: The LPN's will provide and monitor the clinical care for the four residents of the medical group home including planning, assessment, implementation and evaluation. The LPN's will ensure the overall health and safety of the resident and environment in which they reside in. The LPN's will work with the managerial staff in assisting with the clients overall care. The LPN is responsible for the administration of medications as directed by providers. When needed LPN's will assist in doctor appointments.
