Nurse Practitioner Primary Care

Under the direct supervision of the Medical Director and/or Collaborative Physician, the Nurse Practitioner is responsible for quality and completeness of assigned medical services. In accordance with standardized procedures, the Nurse Practitioner is responsible for providing primary care to patients through regular health assessments and the management of common acute and chronic stable conditions. The Nurse Practitioner obtains a health history and performs a physical assessment, identifies problems, and formulates plans that include treatment, health education and counseling. The Nurse Practitioner collaborates with other health care professionals in assessing and managing health care problems and maintaining continuity of care.
Nurse Practitioner Duties and Responsibilities
· Facilitate new patient consultation and health history review
· Perform physical examinations and screenings of patients
· Order and administer tests including bloodwork
· Review and interpret test results to diagnose health problems
· Recommend further treatment options or future appointments if needed
· Provide referrals to other practitioners when necessary
· Provide after-care/ongoing care information and resources to patients and their families
· Ensure patient records are kept up to date including test results, immunizations, condition changes, etc.
Nurse Practitioner Requirements and Qualifications
· Master's degree in nursing
· Current state nurse practitioner license
· 5+ years of nursing experience
· Understands HIPAA and OSHA regulations
· Able to operate as part of a team, alongside physicians and other nurses
