Responsibilities

  • Provide habilitation services to each client we serve as prescribed in their service plans.
  • Assist each client as needed in skill development in areas such as activities of daily living (ADLs), domestic living, recreation, and socialization.
  • Establishing a secure positive environment that nurtures residents’ needs, self-expression, and goals.
  • Advancing residents’ decision-making, productivity, and participation in a range of activities.
  • Promoting independence in daily activities such as work, learning and leisure.
  • Complete required documentation according to standards and policies. To keep Persons Served activity records on a daily basis, recording major activities, behaviors, progress, and other documents as required by ISP or policy.
  • With appropriate training and supervision, administer medication to clients as assigned and in compliance with management, medical and nursing policies, and procedures.
  • Ensure assigned clients’ nutritional needs are met according to prescribed diet and assist with feeding as necessary.
  • Ability to transport clients and serve as their companion while they participate in community activities off-site.
  • Report immediately any observed/known deficiencies or own inability in providing habilitation services to clients as well as all types of abuse and other facility problems, including physical plant/safety issues to management.
  • Ability to lift 50 lbs. independently.
  • Ability to lift or move 175 lbs. with one additional person assisting.
  • Ability to summon emergency personnel on the company’s emergency call list.
  • Ability to maintain necessary medication and CPR certifications.
  • Ability to follow through with and update, in a timely fashion, all other necessary training as outlined by company, state, and federal guidelines.
  • Assist Persons Served or directly provide the following services or supports:
  • Grooming, personal appearance and hygiene.
  • Meal planning and preparation.
  • Money management.
  • Housekeeping (minor yard work and household repairs) and sanitation.
  • Interpersonal relationships.
  • Use of community resources, and leisure resources.
  • Communication and emergency procedures.
  • Health and safety precautions (first aid, universal precautions).
  • Other as necessary.
  • To participate in communication among staff assigned to the location.
  • To inform immediate supervisor of significant concerns/problems/activities within the location / program this may affect Persons Served, families, others, or self.
  • Ability to schedule appointments for Persons Served, i.e. medical, vocational, pharmacy, educational.
  • Participates in IDT and other meetings and development of Overall Plan of Services.
  • Additional duties as assigned.
  • Duties and responsibilities may change, and new ones may be added, at any time with or without advanced notice.

Requirements:

Qualifications

  • Must be at least 18 + years old.
  • High School (HS) Diploma OR GED certificate required (Ability to speak and write English clearly)
  • Strong written and verbal communication skills
  • A valid driver’s license with an acceptable driving record. Must also have current auto liability insurance.
  • Reliable transportation
  • At least one year’s experience providing direct services to developmentally disabled persons preferred.
  • Pass New Mexico Caregivers Criminal Background Check
  • Comfortable with assisting residents with hygiene and medical routines.


*A combination of education and other experience may be considered in lieu of years of experience as DSPs.
