Home and Community Support Staff- DDD

Job Overview
The Home and Community Support Staff provides vital support to individuals in their homes and communities, ensuring they receive the care and assistance needed to maintain independence and improve their quality of life. This role involves working directly with clients to deliver personalized care, support daily living activities, and facilitate community involvement while applying behavior support strategies to manage and promote positive behavioral outcomes.
Key Responsibilities
1. Client Support and Care:
- Provide direct care and assistance with daily living activities, including personal hygiene, meal preparation, accompany clients to their job and medication management.
- Develop and implement individualized care plans in collaboration with clients, families, and healthcare professionals.
- Monitor and report on clients’ health and well-being, adjusting care plans as necessary.
2. Behavior Support Strategies:
- Apply behavior support strategies to manage challenging behaviors effectively and promote positive behavioral changes.
- Utilize techniques such as positive reinforcement, de-escalation methods, and structured routines to support clients.
- Implement behavior support plans (BIPs) with the support of a BCBA.
3. Community Integration:
- Facilitate and encourage client participation in community activities, social events, and recreational programs.
- Assist clients with transportation to appointments, community events, and other activities as needed.
- Build and maintain positive relationships with community organizations and services to enhance client engagement.
4. Documentation and Reporting:
- Maintain accurate and timely documentation of client interactions, care provided, daily activity tracking and any changes in client status.
- Ensure compliance with organizational policies, procedures, and regulatory requirements.
Qualifications
1. Education and Experience:
- High school diploma or equivalent; relevant certifications (e.g., CNA, HHA) preferred.
- Previous experience in home care, community support, or a related field, with a focus on behavior support, is desirable.
2. Skills and Abilities:
- Strong interpersonal and communication skills, with the ability to build rapport with clients and their families.
- Proficiency in applying behavior support strategies and managing challenging behaviors.
- Ability to handle sensitive and confidential information with discretion.
- Capability to work independently and as part of a team in a fast-paced environment.
3. Personal Attributes:

Physical Requirements
- Ability to perform physical tasks related to client care, such as riding bikes, assisting with mobility, and managing household activities.
- Capability to handle varying work conditions, including different home environments and community settings.
Working Conditions
- The role involves working in clients' homes and community settings, which may include different environmental conditions.
- May require irregular hours, including evenings and weekends, depending on client needs.
