*Overview*
We are seeking a dedicated and compassionate Physical Therapist to join our team. In this role, you will be responsible for providing high-quality patient care in various settings, including home health and clinic environments. You will work closely with patients to develop personalized care plans aimed at improving their mobility and overall physical health. Your expertise in orthopedics and medical terminology will be vital in assessing patient needs and implementing effective treatment strategies.
*Responsibilities*
* Conduct thorough assessments of patients‘ physical abilities and limitations.
* Develop and implement individualized care plans that address patients‘ specific needs.
* Provide hands-on physical therapy treatments, including exercises, manual therapy, and modalities.
* Collaborate with healthcare professionals to coordinate discharge planning and ensure continuity of care.
* Educate patients and their families about treatment options, exercises, and preventive measures.
* Maintain accurate documentation of patient progress and treatment outcomes.
* Stay updated on the latest physical therapy practices and advancements in the field.
*Skills*
* Strong knowledge of anatomy and medical terminology relevant to physical therapy.
* Experience in home community care and home health settings is preferred.
* Proficiency in discharge planning processes to facilitate smooth transitions for patients.
* Ability to create effective care plans tailored to individual patient needs.
* Familiarity with orthopedic conditions and treatments.
* Excellent communication skills for interacting with patients, families, and healthcare teams.
* Compassionate approach towards patient care with a focus on improving quality of life.
