We are seeking a dedicated and compassionate Physical Therapist to join our team. The ideal candidate will have a strong background in various settings including clinic, home health, and inpatient care. This role involves developing individualized treatment plans to improve patients' physical function and quality of life. The Physical Therapist will work with diverse populations, including geriatrics and pediatrics, ensuring comprehensive patient care through effective discharge planning and rehabilitation strategies.

*Duties*
* Conduct thorough assessments of patients' physical abilities and needs.
* Develop and implement personalized treatment plans based on patient evaluations.
* Provide hands-on therapy techniques to enhance mobility, strength, and overall function.
* Collaborate with other healthcare professionals to ensure coordinated patient care.
* Educate patients and their families on treatment plans, exercises, and health management strategies.
* Monitor patient progress and adjust treatment plans as necessary to achieve optimal outcomes.
* Maintain accurate documentation of patient evaluations, treatments, and progress notes in compliance with healthcare regulations.
* Stay updated on the latest research and advancements in physical therapy practices.

*Skills*
* Strong knowledge of human physiology and biomechanics.
* Experience in various settings such as clinic, home health, inpatient, geriatrics, pediatrics, and orthopedics is preferred.
* Excellent communication skills for effective interaction with patients, families, and healthcare teams.
* Ability to develop comprehensive discharge planning strategies for patients transitioning from therapy.
* Proficient in utilizing physical therapy systems for documentation and patient management.
* Strong problem-solving skills to address complex patient needs effectively.
* Compassionate approach to patient care with a focus on improving quality of life through rehabilitation.

Job Type: Full-time

Pay: $42.00 - $45.00 per hour

Expected hours: 40 per week
